Job: Discharge Planner

The Discharge Planner plans, develops and organizes the discharge needs of the patients/residents; meets with all patients/residents to review and sign admission paperwork; reviews advance directives as needed with all patients/residents; schedules family meetings as needed to ensure appropriate discharge plans are arranged; and communicates patient needs with home health agencies and managed care insurances.

EDUCATION/WORK EXPERIENCE:

Must possess a Bachelor's degree. Must possess two (2) years of Long Term Care or hospital based subacute experience with significant emphasis in discharge planning. Must possess skills in communication of rehab and medical needs, knowledge of Managed Care Insurance and Medicare.
